What is Propranolol?

Propranolol is a medication used for the treatment of hypertension, angina and tremors. It’s a beta-blocker that helps the blood flow through veins and arteries to improve the circulation and heart. It is often prescribed to help treat heart rhythm disorders and can be prescribed as a preventative measure against heart attack. It can also be used to minimise the frequency and severity of migraines, to help control thyroid or adrenal gland tumours, and is sometimes used as a treatment for performance anxiety. The tablet reduces the workload of your heart so it can beat more regularly without as much effort.

Propranolol is available in four forms – oral tablet, extended-release oral capsule, oral liquid solution and injectable. It is sometimes referred to by brand names including Inderal, Hemangeol and InnoPran XL.   

How does Propanolol work?

Propranolol contains the active ingredient propranolol hydrochloride which is a form of beta blocker. Beta blockers work by reducing the messages sent from the nerves to the heart by blocking beta-adrenergic receptors. This results in a slower heartbeat and less force exerted by your heart which lowers your blood pressure to minimise the problems associated with circulation, heart rhythm and tremors. Propranolol, specifically, competes with the beta-adrenergic receptor-stimulating agents so that the responses are decreased.

Hemangeol (propranolol oral liquid 4.28 milligrams) can be prescribed to infants from five weeks old for the treatment of genetic condition called infantile hemangiomas, which is caused by blood vessels grouping together in an unusual way which can develop into red marks or ulcers on the skin, or lead to complications with the liver, brain or digestive system.

How to use Propranolol

The exact dosage of Propranolol will be prescribed by your doctor, but typically it is taken up to three times per day with or without food. Propranolol should be taken at the same time each day and swallowed whole – do not crush, chew or break open the capsule. Propranolol appears in the bloodstream after 30 minutes of taking and reaches maximum effect between 60 and 90 minutes when administered by mouth.

If being taken for hypertension, take 40mg two to three times per day, as prescribed by your doctor, increasing to 80mg per day at weekly intervals depending on your response rate. The standard dose is between 160 and 320 mg/day. For the treatment of migraines, the typical dose is between 80 to 160 mg/day. For the treatment of angina or tremors, the dose range is typically 120 to 240 mg/day.

It is important to have regular blood pressure checks while taking this medication.

For infants being treated for infantile hemangiomas, doses should be paced at least 9 hours apart and with regular feedings throughout the treatment. The oral liquid should be measured precisely using the syringe provided or with a dose-measuring spoon. Do not shake the Hemangeol liquid.

If you forget to take a dose, take it as soon as you remember unless it’s time for your next dose, in which case just take that dose as normal. Never double up a dose to make up for a missed dose. Do not intentionally skip doses or stop taking the medication suddenly as this may lead to your condition worsening. If you want to come off Propranolol, speak to your doctor about how to safely taper your doses.

What are the side effects of Propanolol?

Common side effects of Propranolol include:

  • Lightheadedness
  • Wheezing, shortness of breath, or trouble breathing
  • Depression, confusion or hallucinations
  • Decreased sex drive, impotence, or difficulty having an orgasm
  • Tiredness and sleep problems
  • Cold feelings in your hands and feet
  • Vision problems
  • Nausea, upper stomach pain, dark urine and jaundice
  • Low blood sugar, leading to sweating, confusion, headaches and hunger
  • Dizziness

 Inform your health care provider immediately if you experience any of the following side effects:

  • Rash or itching
  • Swelling, particularly of the face, tongue or throat
  • Hives

In the rare case of overdose, Propranolol is linked with seizures and cardiac arrest. It should be used with extreme caution in patients who suffer with depression or those with potential suicidal ideation – speak to your GP if you have experience of this mental health disorder.

Before taking propranolol, inform your GP or pharmacist of your medical history. In particular, if you have experience of:

  • Allergic reactions to any ingredients on the patient information leaflet
  • Uncontrollable heart failure or a very slow heart rate
  • Breathing difficulties including asthma, bronchitis or emphysema
  • Low blood pressure or severe blood circulation problems, such as Raynaud’s syndrome
  • Muscle disorders
  • Liver or kidney disease
  • Depression
  • Thyroid disorders

It is not advisable to take Propranolol when you’re pregnant or breastfeeding, as it is not known whether it will harm your unborn baby and it can pass through into breast milk and cause harm to a nursing infant.

Inform your doctor or pharmacist of any medication you’re currently taking, as these could impact how the treatment affects you. In particular, tell your doctor about:

  • Blood thinners, including warfarin, Coumadin and Jantoven
  • Antidepressants, including amitriptyline, clomipramine, imipramine and desipramine
  • Drugs for high blood pressure or prostate disorders, including doxazosin, prazosin and terazosin
  • Heart or blood pressure medicine, including amiodarone, diltiazem, propafenone, quinidine and verapamil
  • NSAIDs, including aspirin, ibuprofen, naproxen, celecoxib, diclofenac and indomethacin
  • Steroid medicine, including prednisone
